Qt 5.8 certification for business needs - which license?

Hi guys,

My company would like to buy for me Qt license.

We are manufacturing company but we are using some applications for visualization of data on PC.
I need license ,,forever", so monthly subscription is unacceptable.

There are plenty of possibilities but to be honest Im not sure what will I need in the future.

I know only that I will be making software for PC with windows and on my PC there is windows too.

Can You help me with issue which license should buy my company for me? (time - unlimited, Windows OS on developer PC, full license, for one person) How much money is it?

PS: Are there any distributor in Poland?
